
13 May Innovative home-based digital care solution changes behavior & positively impacts social determinants of health
Social determinants of health, such as access to healthy foods and exercise opportunities, impact one’s ability to thrive – the earlier one starts, the better. Social determinants of health also impact recovery from surgery and illnesses like Peripheral Artery Disease (PAD), diabetes, cardiac rehabilitation, and many more. Digital health solutions have grown exponentially to tackle chronic conditions.
However, one thing that’s become even clearer this past year is that offering remote care isn’t as easy as it sounds. Why? One in 3 lack the foundational skills necessary to access remote solutions. From digital literacy challenges and lack of live support to confusing medical instructions, it’s no wonder remote care isn’t so simple.
Can a home-based digital care solution overcome these gaps, change behavior and positively impact those social determinants of health? Yes, it can – the data is in.
Our Cell-Ed team is working with medical societies and payors to provide doctors’ offices with remote monitoring and exercise therapy for their patients, facilitated by expert coaches. The app we built combines microlearning with behavior change coaching along with prescribed treatment for patients with chronic diseases.
Data from our programs suggest statistically significant behavior changes, health improvements, and increased program participation.
Here’s how it works: real-life health and wellness courses and live coaching are the building blocks for skills to be developed via short 3- to 5-minute micro-lessons on any device. Activities are tracked, live coaches with health literacy, clinical program, and cognitive-behavioral experience encourage and support clients, and the data is then reported back in real-time so adjustments can be made if needed. Our easy-to-use, accessible programs deliver critical programming while meeting each patient’s personal goals. The virtuous cycle between care team programming, microlearning on the go with coaching, and meeting patient goals is key.
